Discrimination is Against the Law
The Spine Hospital of Louisiana complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), age, disability, or sex (consistent with the scope of sex discrimination described at 45 CFR § 92.101(a)(2)) and consistent with Section 1557 of the Affordable Care Act. The Spine Hospital of Louisiana does not exclude people or treat them less favorably because of race, color, national origin, age, disability, or sex.
The Spine Hospital of Louisiana in compliance with Section 1557 and other federal civil rights laws will provide the following services when necessary to ensure meaningful accessibility and equal opportunity to participate in our programs, activities, services, and other benefits:
Reasonable Accommodations: The Spine Hospital of Louisiana will provide reasonable accommodations for qualified individuals with disabilities. Please inform staff when scheduling your procedure appointment, contact The Spine Hospital of Louisiana’s Registration Representatives during main business hours, or upon arrival inform staff that such services are required.
Appropriate Auxiliary Aids and Services: The Spine Hospital of Louisiana provides individuals with disabilities free appropriate auxiliary aids and services to communicate effectively with us, such as:
- Qualified sign language interpreters,
- Written information in other formats (large print, read aloud upon request by qualified reader, accessible electronic formats, other formats),
Language Assistance Services: The Spine Hospital of Louisiana will provide free language assistance services to people whose primary language is not English, which may include:
- Qualified interpreters, and
- Information translated and written in other languages in both electronic and written form.
If you believe that The Spine Hospital of Louisiana has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with the Chief Nursing Officer (CNO), who will work with the Compliance Officer, in person or by mail, fax, or email. If you need help filing a grievance, you may call 225-768-2760. If you would like to file your grievance in writing, you may present the written grievance to the CNO by faxing it to the CNO at 225-906-4818 or the Compliance Officer at 225-768-2701. You may also contact the Compliance Officer at compliance@spinehola.com.
